Kevin has worked as a Physical Therapist since 1993. He graduated from Washington University School of Medicine. He has worked in long term care, rehabilitation facilities and home health settings. Kevin has the knowledge and expertise to help people return to their best physical health. Kevin was born in Provo, Utah and grew up in Midvale, Utah. Kevin and his wife are the parents of 5 children and have 1 grandchild. Kevin enjoys hunting, fishing and being with his family.
David Gray

David received his Bachelor of Science degree from Utah State University and his Master’s degree from the University of Utah. David has worked in many different environments, including home health, long term care and rehabilitation facilities. He is skilled at assessing the needs of his geriatric patients. David was born in Colorado and raised in Bakersfield, California. He and his wife are the parents of 2 children.
Matt Humphrey

Matt graduated from The University of Texas with a Bachelor of Science degree. He has worked with patients in skilled nursing facilities, in-patient, out-patient, and home health settings since 1993. Matt has been trained in making Wisp Hand Splints and Lymphatic Management. Matt was born in New Jersey and raised in Houston, Texas. Matt and his wife are the parents of 3 children. He loves coaching baseball, being outdoors, hunting and hiking.

Rachel Jenson
Dietician
Rachel graduated from and completed her internship at Brigham Young University. She has been a Dietician since 2001, working mainly in long term care and assisted living facilities. Rachel works with clients to assist them with their specific dietary needs. She and her husband have 2 children and a dog. Rachel loves anything outdoors, including hiking and biking.

Jonathan West
Social Worker
Jonathan West is an Aggie through and through! Born in Logan, Utah but raised in Northeast Ohio, he decided to continue the Aggie legacy. He graduated from Utah State University with his bachelor’s of Social Work where he completed his practicum hours providing the service of the Meals on Wheels program. He completed his Master of Social Work from University of Central Florida in the midst of the pandemic where he provided clinical social work and therapy to adolescents struggling through that difficult time. Since then, Jonathan has become licensed as an LCSW working with a variety of populations from adolescents to Native Americans to seniors. He enjoys spending time with his wife, son, and daughter and sitting down to whatever medium is telling an excellent story.
